Match the report to the offer
Record report ID, issuer, date, site, mill system, cement designation and additive grade/lot. Match the sample label and product document revision to the candidate in the quotation. A supplier case study using another plant or grade is background, not your acceptance result.
State the purchase decision and agreed quality constraints at the beginning. Was the objective energy, output, handling or another quality target? Require measured results and limitations for that objective, rather than treating every favorable number as a separate proven benefit.
Review the comparison, not just the summary

This is an editorial review checklist, not a standard-required reporting format. The responsible plant and laboratory must define the actual method and approval process.
Identify what prevents a fair inference
If composition, separator settings or other operating factors changed along with the additive, disclose those changes. The report may describe a combined optimization, but cannot automatically isolate a chemical-only effect. Do not erase unfavorable intervals while retaining the best result.
Check whether the reference and candidate maintained the agreed cement quality. Faster production accompanied by a different fineness or an unmet quality requirement is not automatically an acceptable purchasing result. Review Blaine, residue and PSD as separate measurements.
Ask what is missing if the report contains only a chart or headline percentage. Request underlying totals or traceable interval data, the calculation and the stated exclusions. If these cannot be supplied, label the conclusion provisional instead of inventing a confidence level.
Audit the commercial conclusion separately
Confirm the chemical-cost calculation from supplied-product mass and the quoted price basis. Energy comparisons need the same meter boundary and compatible production denominator. Keep additional saleable output conditional on demand and downstream capacity; a trial throughput increase is not itself an invoice.

Record the unresolved questions before ordering
Request a signed or released conclusion from the responsible team identifying accepted conditions, unresolved measurements and the next agreed action. Keep the source report with the quotation and grade documents. Record whether the proposed order is a trial quantity or a production purchase; do not blur those decisions.
For planning the actual evaluation use the mill-trial protocol. For IN-5, also retain its grade-specific questions; this generic report checklist does not resolve the supplied-document naming and revision gaps. No report checklist establishes universal dosage, safety or guaranteed energy savings.

Candidate grade, cement/mill identity, traceable baseline and trial report, quote basis, unresolved criteria and destination.
Request the exact grade, current document availability, sample terms, net package mass, quotation validity and delivery terms. Suitability and performance remain subject to applicable documents and agreed evaluation.
